Once in state SND_PCM_STATE_DISCONNECTED an ALSA pcm handle cannot be recovered; the only valid operation is to close it. This commit closes the handle, then attempts to re-open the same device. This allows temporary disconnections to be recovered (eg some devices when resuming from suspend require this).
Sending silence / dropping recieved frames when the stream is corked needlessly wastes battery charge for bluetooth devices. Limit the silence/drop time to 10 seconds, after which the pcm is closed; then re-open the pcm when the stream is uncorked.
